Is it possible to offer a discount/coupon for the first time a visitor/customer orders a particular item? The next time they return and buy the item it does not discount it. I can just think of having a popup with a coupon code in it and only popups when a cookie is not found on the connecting device. The coupon would need to be change periodically to prevent repeated use. Is there a way to offer a coupon/discount for first time customers? perhaps if they create an account for the first time they get a discount? version 7.0.1

This would inevitably be based on them creating a customer account, no? In that case, how many signups are you going to get? Would manually sending an email when customers sign up be an option?

I believe that would be the best way. When a customer creates an account the discount can be at the login level. That's fine for new customers. However, when they place a second order the discount would still apply at that login level unless you adjusted their login level after their first order that contained the sale item, or adjusted the sale item to a different login level periodically. This would be a lot of work to do. Existing customers could be sent a one-time use coupon code for the sale Item I suppose. Its the new customers that is tricky to give them the discount at the time of purchase on their very first order. The ECT needs a mechanism to allow promotions based on email address so at the time of checkout it checks the email address against the promotion to see if it already was used. I guess there is just no good way to do this.
